Lead Social Prescriber, Rosebank Health PCN
Hi I’m Abi and I’m the lead social prescriber at Rosebank Health. I’m also an advanced ambulance practitioner with SWAST.
I’ve worked within the NHS for the past 26years & over time have seen many processes come and go (and also re-appear again!)
Social prescribing brings together my passion for personalised care, my clinical knowledge and my desire to want to help my patients so that they can help themselves to feel better

Matron Manager, Adult Complex Care at Home Team
Hi, I am Gail and am the service lead for Complex Care at Home in Gloucester, Cheltenham and the Forest of Dean. I have been a nurse for 24 years and have worked in primary care and the community for 21 of those years! It seems such a long time but I still learn something new every single day. The majority of my career has been spent working with people living with Diabetes in Gloucestershire and Worcestershire and I moved from specialising in Diabetes in 2018 into Complex Care, where we support people living with frailty, complex health conditions and social care needs. I have been on quite a journey developing the service in 3 localities and have an amazing, skilled and compassionate team who provide truly personalised advance care planning, treatment and support to patients, their families and carers who are referred into our service.
